Certification and Authorization

I hereby authorize First Commonwealth Mortgage to obtain a consumer credit report through a credit reporting company chosen by First Commonwealth Mortgage.

I understand and agree that First Commonwealth Mortgage intends to use this consumer credit report for purposes of evaluating my financial readiness to buy or refinance a home.
